DESCRIPTION

The Submandibular is the second in size gland that produces saliva and the first one in produced quantity. It is found very close to the neck mid-line, lower and behind the chin. In case of a lithiasis or a tumour or a cyst in the area, then the whole gland is being removed with its duct, anong with the damaged area. The usual symptoms are the augmentation of the gland after a meal and maybe pain and sensitivity on this area. Before the operation we can do: ultrasound examination, CT scan, MRI scan and FNA (Fine needle aspiration biopsy)

The operation

Resection of the submandibular gland along with the stones or tumours included and sending for histological examination. If lymph nodes have also been affected there may be lymph nodes dissection as well (several different types). This can be done after having quick biopsy results or without them

Stages

  1. Incision made along a natural skin crease about 4 cm below the lower edge of the horizontal portion of the mandible for a better visual effect and avoiding injury ro the mandibular branch of the facial nerve
  2. Identification of the submandibular gland
  3. Resection of the gland, with the stones (or tumours) included
  4. If lymph nodes have also been affected there may be lymph nodes dissection as well (several different types). This can be done after having quick biopsy results or without them
  5. Sending the whole extracted material for histological examination
  6. Suturation of the incision and placement of a small tissue drain

Combination with other operations

It cannot be combined with other operations apart from lymph nodes dissection, since this is required at this stage of the disease or has been decided by the operation findings

Instructions after surgery

  • Mobilization of the patient the afternoon after surgery
  • Removal of the small tissue drain in 1-2 days
  • Removal of the incision sutures within a week
  • Painkillers are provided in small doses and only if there is a request from the patient. In general terms, this is not a painful operation, but you may feel some kind of "twitching" on the operated area
  • Antibiotics are being given as a preventive measure for 7-10 days
